Junior League of Oklahoma City

I understand and agree to the placement requirements.

1. I understand that all orientation(s) and/or training session(s) for the placement are mandatory.
2. The Junior League of Oklahoma City, (JLOC) is not liable for any personal injury or property damage suffered by its volunteers. I hereby agree to assume the risks involved in this volunteer placement and to hold the JLOC harmless in the event that I am injured or suffer property damage. Additionally, I understand the insurance policies of the JLOC do not provide liability or worker’s compensation coverage for the benefit of its volunteers. I understand that I must consult with my personal insurance representative to ensure my coverage is adequate.
3. I have read the Bylaws and Policies of the JLOC and I agree to abide by them. These documents are available for review on the Resources tab of the Members Only section of the website.
4. I consent to the taking, recording, use and publication of my photograph and/or audiovisual records and images by the JLOC in any of its written, electronic and audiovisual materials and publications, including, but not limited to, magazines, newsletters, programs, website, social media postings, advertisements, brochures and videos. If you do not consent to the taking, recording, use and publication of your photograph and/or audiovisual recordings and images by the JLOC in the manner stated above, you must contact headquarters in writing of your intentions and include a photograph.  The JLOC will use the photograph for identification purposes and hold it in confidence.
5. I release the JLOC, its officers, directors, members, employees and agents from any and all claims of any type, which I may hereafter have based upon the taking, recording, use and publication of the photograph, electronic or audiovisual records or images of me. This release is binding upon me and my successors and personal representatives.
